The first email can be blank if you do not want to state anything. It will be forwarded and you will be contacted by a Doctor.

There may come a point where treatment is required and/or real names will be needed in order to proceed. This is a decision that is entirely up to you. You must also bear in mind that direct consultations and/or treatment may require payment.
